Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY), is a pioneer Healthcare programme initiated by the Honourable Prime Minister on 23rd September 2018 with objective is to provide Adequate and Free Health cover to the vulnerable sections of society especially economically weaker citizens. 

Features of Ayushman Bharat Yojana 


1) To provide affordable healthcare to all especially the vulnerable sections of society.


2) To provide free medical treatment up to Rs. 5 lakhs per year for each eligible individual.Covers the costs of medical treatment, diagnostics, medicines, pre and post hospitalisation expenses. 


3) It is a paperless & cashless Health Insurance Scheme available at all government hospitals and select private hospitals. 


 

How To Apply For Ayushman Bharat Yojana ?


Ayushman Bharat Yojana scheme is offered to all eligible beneficiaries hence there is no separate enrolment process. 


Step 1:  

Check your Eligibility


for eligibility check visit an Empanelled Health Care Provider hospital near you. Ask  at the registration desk to about your eligibility  or Visit any PM-JAY kiosk to check your eligibility or  Call the helpline numbers 1800-111-565 or 14555 to check your eligibility. 


Step 2 :

visit official website “pmjay.gov.in” and select the “Am I eligible” button. 


Step 3 :

Enter your mobile number, type the correct the captcha code, and generate OTP.  


Step 4 :

Select your state and check for your name on the beneficiary list by entering your mobile number, ration card number, or HHD number.


Step 5 :

Once you provide the required details, you can get the results stating if you’re eligible for PM-JAY or not. Once you have checked your eligibility for the scheme, the next step is to generate the e-card. It can be done either online or offline


Step 6 :

In case if you are eligibile generate the Patient Card  (PM-JAY e-card) which is paperless, cashless scheme to avail  benefits at any network hospital in India, beneficiaries should provide the PM-JAY e-card during hospital admission. This e-card contains all the information about the patient and is applicable at all empanelled hospitals across India.


Online Process Patient Card  (PM-JAY e-card )

To generate the PM-JAY Golden card online, Visit the official PM-JAY website (mera.pmjay.gov.in).enter the registered mobile number Generate your HHD code (Household ID number). The rest of the process has to be completed offline.by providing HHD code to the CSC (Common Service Centre) Ayushman Mitra representatives, will help you with the rest of the process to obtain card to be paid  sum of rupess thirty .Each member of the beneficiary’s family is provided with a separate patient card. 




Offline Process to generate Patient Card  (PM-JAY e-card )

visit a nearby PMJAY kiosk which  are set up at government hospitals, district collector offices, etc. The PMAM (Pradhan Mantri Arogya Mitra) operating the booth will verify your identity and eligibility using the BIS (Beneficiary Identity System). This is when your e-card (PM-JAY Golden Card) will be generated. 


Documents required for the enrolment :


1) Aadhaar card for identity and age proof 

2) Caste and income certificate 

3) Residential proof using ration card 



What is covered under Ayushman Bharat Yojana? 

1) coverage of a maximum of Rs. 5 lakhs to eligible beneficiaries which includes Medical examination, diagnostic tests, consultation, medical treatments, implants, intensive care, non-intensive care services, physician fees, and surgeries.

2) Pre-hospitalisation expenses for up to 3 days

3)Post-hospitalisation expenses for up to 15 days 

4) Hospital room rent, food services, etc. 

5)  Covid-19 treatment 

6)All pre-existing conditions are treated from day one



Bottom Line : There are no restrictions based on the size of the family or the age of family members. Once you have received your Ayushman Bharat Patient Card and avail the medical services at any public and private hospital across the country.
